Director of Quality Improvement 

DIVISION:     Area Administration

 

QUALIFICATIONS

 

Education

  • Master's degree in Education, Human Services, Business Administration or related field and three years of related experience or a Bachelor's degree (B.A.) in the above mentioned areas and five years experience.

 

Skills

  • Analytical skills necessary to assist staff in developing creative solutions to problems identified in services and/or areas in need of improvement.

  • Interpersonal skills necessary to interact with and train personnel at all levels within the agency.

  • Preparation and negotiation of contracts with outside providers.

  • Knowledge of federal, state and county legislation/ regulations pertinent to agency-wide staff responsibilities.

  • Active participation in a multi-disciplinary team.

  • Oral and written communication in an organized and articulate manner.

  • Assessment of agency-wide Corporate Compliance and training needs.

MAJOR FUNCTIONS

 

Corporate Compliance

  • Coordinate activities, which ensure the agency's provision of quality services, promotion of continuous quality improvement and oversee and monitor agency's compliance with all applicable regulations.

  • Chair Area Corporate Compliance Committee meetings, and represent Area at Diocesan Corporate Compliance Meetings.

  • Perform chart audits, site audits, etc. as needed.

  • Assist programs in establishing ongoing objectives and goals for the services they provide.

  • Identify services or areas in need of improvement and assist staff in developing ways to meet standards.

  • Assist in the collection, analysis, review and reporting of data for quality improvement activities.

  • Monitoring systems to ensure compliance with these regulations, ex. Medicare, Medicaid, Third Party.

  • Meet quarterly with program directors to review and revise quality initiatives.

  • Work closely with staff, committees in meetings or on special projects as needed to ensure proactive approach to quality care of services.

 

Education and Training

  • Assess agency-wide staff training needs by conferring with Directors, managers, and supervisors.

  • Serve as agency point of contact for prospective student interns, and make appropriate referrals.

  • Recruit internal and external trainers and provide trainers with identified learning objectives for workshops.

  • Select appropriate instructional procedures or methods such as individual/ group instructions, lecture, self-study, computer based training, etc.

  • Conduct   and   facilitate  ongoing   training  opportunities     including    new employee orientation.

  • Keeps informed on new developments, methods and techniques in training field.

 

Regulatory Compliance

  • Keep informed of all health and safety regulations and insure agency compliance, meeting all time frame deadlines.

  • Coordinate and facilitates all mandated trainings including first-aid, CPR, sexual harassment, blood borne and airborne pathogens, defensive driving, etc.

  • Completes and maintains related training records for regulatory bodies.

  • Ensures Agency compliance with OSHA standards and other regulatory bodies.

 

Administrative

  • Required to follow all Agency policies and procedures, using the Policy and Procedures manual as a reference.

  • Required to understand and abide by those laws, regulations, policies and procedures that are applicable to the

  • Required to report any known or suspected violation of laws, regulations, policies and procedures related to programs/services of the agency.

  • Required to maintain accurate statistical records, and files on program participants and activities, including but not limited to: individual client records, billing records, medication records, daily log, expense sheets, purchase orders, petty cash, client funds, mileage records, and menus.

  • Required to ensure that all employees are aware of and abide by those laws, regulations, policies and procedures that are applicable to the program/service, which they are providing.

  •    Receive reports of known or suspected violations and investigating them.

  •    Takes no retaliatory action against employee reporting, a known or suspected violation.

 

  • Organizes and develops training manuals, reference library, multimedia visual aids and other educational materials.

  • Trains assigned instructors and supervisory personnel in effective techniques for training.

  • Formulates training policies, programs and schedules, based on knowledge of identified training needs, agency program processes and administrative systems.

  • Maintains records and prepares statistical reports relevant to trainings.

  • Oversee maintenance of training database for all agency staff.

  • Conduct and analyze periodic surveys/consumer feedback tools for assessment and research purposes.

Chief Human Resource Officer

DIVISION:     Area Administration

  

QUALIFICATIONS

 

Education

Bachelor's degree (B.S.) in Personnel Administration, Business Administration, or a related field from a four-year college or university and seven years management related experience. Three years of this experience must be in a Human Resource management capacity or similar administrative position.

 

Experience

Demonstrated ability to provide:

Experience in benefit management.

Thorough knowledge of employment laws, including but not limited to OSHA standards, labor laws, and related health regulations.

Experience in supervising clerical/secretarial staff.

Communicate orally and in written form in an organized, concise manner.

Ability to obtain and analyze data related to Personnel issues.

Experience working with ADP or similar HR related software.

Executive Director

 

MAJOR FUNCTIONS

 

BENEFITS

Directs implementations and administration of benefit programs designed to insure employees against loss of income due to illness, injury, layoff, or retirement.

Enrolls new employees in benefit plans and provides benefit orientation.

Maintains employee benefit files and other record keeping.

Maintains required documentation of insurance coverage, pension plan, disability and other related benefits.

Oversees the record maintenance for employee accrued vacation and sick time.

Directs preparation of and distribution of written and verbal information to inform employees of benefit programs, such as insurance plans, pension plans, etc.

 

REGULATORY COMPLIANCE

 

Keeps informed of labor law changes or diocesan policies which have an impact on the personnel practices of the Agency and ensures that proper steps are taken to carry out these changes.

Completes and maintains records of employee characteristics for governmental reporting purposes.

Maintains confidential employee records as they pertain to exposure to blood borne and airborne pathogens, while keeping the Agency in compliance.

Ensures Agency compliance with OSHA standards and other regulatory bodies, coordinating all exposure control employee medical inoculations and testing as mandated.

 

ADMINISTRATIVE

 

Serve as the Coordinator of the Area Board Personnel Committee.

Represent the Agency at personnel related hearings and investigations, such as Human Rights reviews, etc.

Administer United Way Program.

Serve on the Executive Committee of the Agency Tax Sheltered Annuity.

Convene and facilitate conflict resolution panel when required.

Serve as consultant to Executive Director on policies, laws, etc.

Regularly provide Accounting Office with current employee salary & benefit information.

Responsible for providing initial salary & fringe benefit information for budget preparation.

Serve as Agency designated point of contact for reporting alleged sexual harassment.

Serve as the point of contact for the Diocese HR department.

 

DOCUMENTATION

 

Investigate personal injury/accidents and prepares required documentation for insurance carriers and Diocese.

Prepares employee separation notices and related documentation, and conduct exit interviews, compile data and prepare analysis for Executive Director.

Administers annual employee appraisals and maintains required documentation.

 

EMPLOYEE RELATIONS

 

Make Referrals to the Employee Assistance Program.

Makes recommendations to supervisors when disciplinary actions are in question.

Maintenance Worker

Maintenance Worker position available within Facilities Management.

 

A minimum of a high school diploma or equivalent and a valid NYS driver license is required. Successful completion of training in the field of plumbing, electrical or carpentry, and a minimum of 3 to 5 years related maintenance experience is also required. The ability to respond to calls after regular work hours is also required.
